This Privacy Policy explains how Leviant LLC, doing business as Katy Medical Complex Pharmacy ("Katy Medical Complex Pharmacy," "we," "us," or "our"), collects, uses, and protects your information when you visit katymedicalpharmacy.com, use our pharmacy services, or take part in our text messaging (SMS) program.

• Contact information: your name, mobile phone number, email address, and mailing address.
• Identity information: your date of birth and, if you choose to provide them, images of your identification documents uploaded through our secure link, used to verify your identity for pharmacy services.
• Text messaging information: your mobile number, your text message consent, the date, time and method of your opt-in, and your opt-out requests.
• Technical information: basic device and usage data, such as browser type and pages visited, when you use our website or open links we send you.

2. How we use your information

• To provide pharmacy services and respond to your questions.
• To send one-time verification codes when you sign in to your account.
• To notify you when an order is ready for pickup.
• To send secure links for uploading identification documents.
• To follow up on your visit and, occasionally, invite you to leave a review.

3. Health information

Prescription and health information you give the pharmacy is protected health information under the Health Insurance Portability and Accountability Act (HIPAA). Our text messages never include medication names or details about your health.

4. Text messaging (SMS)

Katy Medical Complex Pharmacy sends text messages only to people who have opted in, either on our sign-up form at katymedicalpharmacy.com/opt-in (also used on the tablet at our pharmacy counter) or by giving verbal consent to our staff. We use your mobile number only to send the messages described in our SMS Terms.

Message frequency varies. Message and data rates may apply. Reply STOP at any time to stop receiving messages, or HELP for help.

5. How we share information

We do not sell your personal information. We share information only:

• With service providers that help us run our services, such as our text message delivery and secure hosting providers, only to provide those services to you and under confidentiality obligations. This never includes sharing text messaging opt-in data or consent for any marketing purpose.

We use administrative, technical, and physical safeguards, including encrypted connections and restricted access, to protect your information. No method of transmission or storage is completely secure, but we work to protect your information and review our safeguards regularly.

We keep personal information only as long as needed for the purposes in this policy or as required by pharmacy and healthcare record-keeping laws. We keep records of opt-out requests so that we continue to honor them.

• Text messages: reply STOP to any message to opt out. You will receive one confirmation message and no further texts unless you opt in again. Reply START to re-subscribe.
• Access and corrections: you may ask to review or correct the contact information we hold about you by contacting us below.

9. Children's privacy

Our website and text messaging program are not directed to children under 13, and we do not knowingly collect personal information online from children under 13.
